Firstly you will have to have already followed the steps as instructed by Andy here

Self employed / company owners:- Next you can email a copy of your letter head, public liability insurance and 2 statements / invoices from trade suppliers. Also make sure you include you real name so we can tie it up to your status application.
Employees:- Email 3 months wage slips from an employed position in the trade along with the name / number / email address of your manager / personel dept.
